Hospital/Homebound Services

Hospital/Homebound (HHB) Instruction is defined as: “academic instruction provided to students who are confined at home or in a health care facility for periods of time that would prevent normal school attendance based upon certification of need by the licensed physician or licensed psychiatrist who is treating the student for the presenting diagnosis.” When a student is absent for ten days, or if there is knowledge that the student will be absent for ten or more days, the student’s parent or guardian must obtain from the Student Services Offices, an agreement concerning HHB policies and procedures, parental cooperation, and release for medical information relating to the reason for the request for HHB services. The medical release form and the referral for HHB services must be signed by the physician and parents, and then submitted to the Student Services Office. Within five school days of receiving the completed forms, the school will provide written notification of the time and place of the school team meeting, if for general education students, or the IEP meeting, if for students with disabilities, regarding HHB services. At the meeting, the team will develop an Educational Service Plan which will include accommodations and modifications through the Pyramid of intervention, Section 504 plan, or Individualized Education Program (IEP) and school reentry procedure upon return to school.

Please note that some students who have recurring or chronic health conditions may need to have a 504 plan or may need to be considered for Other Health Impaired services. The 504 plan or an IEP can address intermittent, chronic and recurring school absences without having to have a physician's referral each time. These cases will be handled differently than those that are one-time occurrences.
